Client Consent Agreement

Please review this agreement before your dog's first day of daycare.

  1. I (The Owner) understand that if my dog is deemed too dangerous for daycare services, Beegan K9 Solutions LLC reserves the right to refuse service.
  2. I expressly waive and relinquish any claims against Beegan K9 Solutions LLC, its employees, and representatives, except those arising from negligence on the part of Beegan K9 Solutions LLC.
  3. I understand that I am liable for any medical care expenses and damages that result from injuries caused by my dog, except those arising from negligence on the part of Beegan K9 Solutions LLC.
  4. If any medical problems develop while my dog is in the care of Beegan K9 Solutions LLC, I authorize Beegan K9 Solutions LLC to do whatever they deem necessary for the safety, health, and well-being of my dog. Further, I agree to assume full financial responsibility for all expenses incurred.
  5. I agree that in the event of a medical emergency or if asked for medical procedures, Beegan K9 Solutions LLC may have my dog treated by my (The Owner's) veterinarian of choice, or if not provided, by a veterinarian of Beegan K9 Solution LLC's choosing, and I will be responsible for the cost of any medical treatment.
  6. My dog is on Heartworm medication and Flea & Tick preventatives. If a dog that is dropped off at Beegan K9 Solutions LLC's facility for daycare and is found to have fleas and/or ticks, it will be quarantined, bathed, and given proper medications at the owner's expense, or otherwise be sent home.
  7. My dog has up-to-date vaccinations. The documents I am providing to Beegan K9 Solutions LLC show proof of current DHLPP (Distemper, Hepatitis, and Parvovirus) and Rabies vaccinations.
  8. I consent to Beegan K9 Solutions LLC's use of my dog in social media content and promotional posts.
  9. Updated pickup times for daycare services are provided at https://beegank9.com/. Unless other arrangements have been agreed upon by Beegan K9 Solutions LLC, I hereby agree to incur a $15 fee if I am late for pickup.
  10. I hereby declare to Beegan K9 Solutions LLC that I am the legal owner of my dog; that my dog has not been exposed to Distemper, Rabies, Parvo, or Canine Influenza within the past (30) thirty days; that my dog has been inoculated as indicated by records presented; that my dog is currently and properly licensed; and that I (The Owner) have read this agreement in its entirety.
